Emotion regulation difficulties of patients with bipolar disorder and first-degree relatives

Abstract
Objective: Difficulty in emotion regulation is the deficiency to regulate an individual's emotional response to a negative stimulus. Our aim is to investigate the effect of emotion regulation difficulty on the process and function of bipolar disorder (BD). Methods: 64 patients with BD, 64 patient’s first-degree relatives and 66 healthy controls were included in the study, which were followed in Mersin University Research and Practice Hospital Outpatient Clinic of Psychiatry. Sociodemographic Information Form and Difficulties in Emotion Regulation Scale (DERS) were filled to all participants. In addition; SCID-I, Short Form-36 (SF-36), Hamilton Depression Rating Scale (HDRS) and Young Mania Rating Scale were filled to the patient group. Statistical analyses were performed with SPSS 21.0 computer program. Results: DERS scores in patients’ group were found to have significantly higher scores than patient’s relatives and healthy controls (p<0.05). But no significant relationship was found between relatives and healthy controls (p>0.05). Another finding that who score high scores on DERS receive lower scores on SF-36. In depressed patients, there was a significant positive correlation between HDRS scores and DERS scores. Discussion: In our study, it was shown that difficulty of emotion regulation negatively affected the functioning and disease course of BD patients and there is a positive correlation between difficulty in emotion regulation and impairment in functioning in depressive episodes

Bipolar bozukluk hastalarının ve birinci derece yakınlarının duygu düzenleme güçlükleri

Ozet
Amaç: Duygu düzenleme güçlüğü olumsuz bir uyaran karşısında bireyin duygusal yanıtının düzenlenmesinde yetersizlik olmasıdır. Amacımız, duygu düzenleme güçlüğünün bipolar bozukluğun (BB) gidişi ve işlevselliği üzerine etkisini araştırmaktır. Yöntem: Mersin Üniversitesi Araştırma ve Uygulama Hastanesi Psikiyatri Polikliniği’nde izlen-mekte olan 64 BB hastası ve 64 hasta yakını ile diğer polikliniklerde başvuran 66 sağlıklı kontrol grubu çalışmaya alındı. Tüm katılımcılara Sosyodemografik Veri Formu ve Duygu Düzenleme Güçlüğü Ölçeği (DDGÖ); ek olarak hasta grubuna SCID-I, Yaşam Kalitesi Ölçeği (SF-36), Hamilton Depresyon Derecelendirme Ölçeği (HDDÖ) ve Young Mani Derecelendirme Ölçeği (YMDÖ) uygulandı. Sonuçlar SPSS 21.0 programıyla istatiksel olarak analiz edildi. Bulgular: Çalışmamızda hasta grubunda DDGÖ puanları hasta yakını ve sağlıklı kontrollere göre istatistiksel olarak anlamlı düzeyde daha yüksek saptanmıştır (p<0.05). Hasta yakınları ile sağlıklı kontroller arasında anlamlı farklılık bulunmadı (p>0.05). Ayrıca DDG֒den yüksek puan alanların SF-36’da daha düşük puanlar aldığı saptandı. Depresif nöbette olan hastaların DDGÖ ve HDDÖ puanları arasında anlamlı düzeyde pozitif ilişki saptandı. Tartış-ma: Çalışmamızda duygu düzenleme güçlüğünün BB hastalarının işlevselliğini ve hastalığın gidişini olumsuz yönde etkilediği, depresif nöbette duygu düzenleme güçlüğü ile işlevsellikte bozulma arasında pozitif ilişki bulunduğu gösterilmiştir.
