Evaluation of the relationship between autistic spectrum disorders with Lyme disease

Abstract
Objective: Autistic spectrum disorders (ASD) are chronic neurodevelopmental disorders leading to deterioration in social, communicative and behavioral skills. Etiology and pathogenesis of the disease has not been elucidated fully yet. In this study, the aim is to investigate the relationship between the Lyme disease with ASD. Methods: Forty children, aged between 3 and 18 years and their mothers participated as the study group and the patients’ 18 healthy brothers participated as the control group in this study. The groups compared clinical and serological for Lyme disease. Autism severity was assessed by Childhood Autism Rating Scale (CARS). Serological evaluations was applied with EnzymeLinked ImmunoSorbent Assay (ELISA) and Western Blot (WB) tests. Results: All parti-cipants’ serums was analyzed by ELISA for Borrelia burgdorferi antibodies and WB test was applied to serums with positive and border levels. WB IgM antibody levels were determined negative for all four patients and one healthy control. WB IgG test performed in two patients, the result was found negative for one of them and one was border. The WB IgM test was made for one mother and it was found negative, WB IgG test was performed for six mothers and the results were found negative for four (66.7%) of them and two (33.3%) were border. No statistically significant difference was found between WB test results which applied for patients and control group. Discussion: Although tick-borne infections are common in Turkey, studies investigating the relation-ship between autism and Lyme disease have been limited. This study is one of the studies investigating the relationship between the Lyme disease with ASD. In our study the ASD patients, their mothers and healthy brothers as the control group was evaluated and Lyme disease was not detected in any participants serologically and clinically.

Otistik spektrum bozuklukları ile Lyme hastalığı arasındaki ilişkinin değerlendirilmesi

Ozet
Amaç: Otistik spektrum bozuklukları (OSB) toplumsal, iletişimsel ve davranışsal becerilerde bozulmalarla giden, kronik gidişli nörogelişimsel bozukluklardır. Etiyolojisi ve patogenezi henüz tam olarak aydınlatılamamıştır. Bu çalış-mada OSB etiyolojisinde rolünün olabileceği öne sürülen Lyme hastalığı ile OSB’nin ilişkisinin incelenmesi amaçlan-mıştır. Yöntem: Bu çalışmada OSB tanısı konan ve izlenmekte olan 3-18 yaşları arasındaki 40 çocuk ve anneleriyle, sağlıklı kontrol grubu olarak alınan OSB’li çocukların 18 kardeşinde serolojik ve klinik olarak Lyme hastalığı pozitifliği olup olmadığı karşılaştırılmıştır. OSB’nin şiddetini belirlemek amacıyla Çocukluk Otizmi Derecelendirme Ölçeği (ÇODÖ) uygulanmıştır. Serolojik incelemeler Enzyme-Linked ImmunoSorbent Assay (ELISA) ve Western Blot (WB) testleriyle yapılmıştır. Sonuçlar: Tüm katılımcıların serumlarında ELISA ile Borrelia burgdorferi antikoru bakılmış, pozitiflik veya sınır düzey saptanan serumlara WB testi yapılmıştır. WB IgM testi ile antikor düzeyi ölçülen dört hastanın ve bir sağlıklı kontrolün hepsinin sonucu negatif saptanmıştır. WB IgG testi yapılan iki hastanın birinde sonuç negatif, birinde sınırda bulunmuştur. WB IgM testi yapılan bir (%2.5) annede sonuç negatif, WB IgG testi yapılan altı annenin ikisinde (%33.3) sınır, dördünde (%66.6) negatif sonuç saptanmıştır. Hasta ve kontrol grubuna uygulanan WB testinin sonuçları arasında istatistiksel olarak anlamlı fark bulunmamıştır. Tartışma: Türkiye’de kene kaynaklı enfeksiyonların sık görülmesine karşın otizm ve Lyme hastalığı arasındaki ilişkinin araştırıldığı çalışmaların az olduğu görülmüştür. Bu çalışma da Lyme hastalığı ile OSB’nin ilişkisini araştıran az sayıdaki araştırmadan biridir. Çalışmamızda OSB’li hasta, anneleri ve sağlıklı kontrol olarak kardeşleri değerlendirilmiş; serolojik ve klinik olarak hiçbir katılımcıda Lyme hastalığı saptanmamıştır.
